PURPOSE AND SCOPE

To provide Clinical treatment services to an assigned population and caseload for mental health and addictive disease to work toward a life of recovery. To collaboratively work with a multidisciplinary team, internally and externally, to determine treatment, resources and needs for assigned caseload.

DUTIES AND RESPONSIBILITIES

1. Completes behavioral health assessment and treatment services for assigned caseload to determine level of treatment needs.

2. Treatments services include, but not limited to, assessment, counseling, treatment plan review, transition/discharge planning, crisis intervention and response.

3. Provides case management services related to identified treatment needs.

4. Refers individual served or family to community resources to assist in recovery from mental or physically illness and to provide access to services such as financial assistance, legal aid, housing, job placement or education.

5. Performs commitment determinations and/or execute orders of involuntary commitments.

6. Works collectively with a team of multidisciplinary professionals, internally and externally, to properly treat assigned caseload.

7. Will travel and provide services at the designated location for the mobile unit each day.

8. Will work in collaboration with care coordinator assigned to mobile unit daily to assist individuals with connecting to telemedicine services provided from the mobile unit.

9. Follows all policies and procedures of SBHS and insures application of all program requirements.

Assists in the planning, development, scheduling and implementation of:

1. program activities, as assigned.

2. Acts as a liaison/representative with stakeholder in a variety of settings/events locally and statewide.

3. Acts as advocate for individual served in order to coordinate required services or to resolve crisis.

4. Performs job responsibilities with minimal supervision.

5. Attends on and off site trainings, with light travel, as assigned.

JOB REQUIREMENTS:

 QUALIFICATION, EDUCATION AND TRAINING

  • Master’s degree in a related field and two years’ experience performing responsibilities in health care services equipment.
  • Licensed Independent Practitioner; or receiving within six months of offer.

PAY

  • $70,000.00 - $75,000.00 per year
